OWNER AGREEMENT

 

 

 

 

 

I, ________________________________, hereby certify that my dog(s) is (are) in good health and has (have) not been ill with any communicable condition in the last 30 days.

 

 

 

 

 

I hereby certify that my dog(s) has (have) not harmed or shown aggressive or threatening behavior towards any person or any other dog.

 

 

 

 

 

I understand and agree that in admitting my dog(s) to Flying High Pet Resort all staff has relied on the above representation regarding the health and behavior of my dog(s).  I further understand and agree that I am solely responsible for any harm caused by my dog(s) to another dog or staff member while my dog(s) is/are attending Flying High Pet Resort.  I agree to release Flying High Pet Resort staff of any liability for any problems which may develop, provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dog(s) attendance and participation at the resort.

 

 

 

 

 

I understand and agree that any problems which develop with my dog(s) will be treated as deemed best by staff of Flying High Pet Resort at their sole discretion, and that I assume full financial responsibility for any and all expenses involved.

 

 

 

 

 

I further understand that Flying High Pet Resort reserves the right to refuse admittance to any dog for any reason, and that no aggressive dogs will be allowed to attend the resort.  Due to insurance restrictions Flying High Pet Resort reserves the right to refuse admittance to pit bulls and pit bull mixed breeds.

 

 

 

 

 

I certify that I have read, understand, and agree to the policies, conditions, and statements of Flying High Pet Resort as set forth in this Owner Agreement, including the following:

 

 

 

 

 

DAYS AND HOURS:  Daycare and Boarding are available year round.  Reservations for daycare are recommended and reservations for boarding are required.  Daycare hours are 7 am to 6:30 pm Mon-Ffri and 9 am to 4 pm Sat & Sun.  Any dog that has not been picked up from daycare by the end of day care hours will be charged a $10 late fee.  Any dog not picked up from daycare before 30 minutes after daycare hours will be considered to be a boarding dog and will charged accordingly.  Grooming is available by appointment only.

 

 

 

 

 

RESERVATIONS:  Reservations are required. 

PAYMENT:  Cash, Checks, Visa and Mastercards are accepted.  A 2% processsing fee will be applied to all credit card transactions.  All payments are due at time of service.  A $1.00 late fee will be added to any unpaid blances up to the day of payment.

CANCELLATION POLICY:  We require 24 hour cancellation notice for boarding dogs.  If 24 hour notice is not given, an overnight fee will be applied to the owner's credit card on file.
